Appointment Scheduling
We seek to accommodate the needs of our patients and will make every effort to schedule your appointment at the most convenient time possible. We strive to offer same day appointments with one of our two physicians. If you have more than one medical issue to discuss with your physician please tell the scheduler so additional time can be arranged.
Insurance and Demographic Information
We see and provide care to patients with any insurance plan or who have no insurance. However, we only participate with Blue Cross and Blue Shield of NC, Cigna, and Medcost insurance plans at the current time. We provide care to Medicare patients as a non-participating provider and do not accept assignment. If you are not insured by a plan that we do business with, payment in full is expected at each visit. If you are insured by a plan we do business with, but don’t have an up-to-date insurance card, payment in full for each visit is required until we can verify your coverage. Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you may have regarding coverage. Prescription Refills
Refills should be requested through your pharmacy. They will fax us a request that includes all necessary information. Refills will be completed within 24-48 business hours.
Test Results
Test results may take up to a week. Your physician or her assistant will contact you by telephone or by mail with your test results once they have been received. If you have not been contacted within a week of your test or procedure, please call our office.
Phone Calls
We make every effort to handle clinically-related calls within 24 business hours. Calls will be returned by either Dr. Daniels, Dr. Peck, or one their assistants. In order to expedite your request, it is important that you provide complete information when leaving a message with our staff.
No-Show Policy
It is the responsibility of patients to arrive for their appointments on time. Due to our limited staff, we are unable to provide reminder calls to every patient prior to the appointment. As a courtesy, we do attempt to contact individuals scheduled for a complete physical exam. Cancellations must be received 24 hours in advance, to allow us to accommodate other patients who need to be seen. Patients who do not contact us prior to their appointment will receive a no-show charge which ranges from $25 for a standard office visit or $75 for a complete physical exam.
Completion of Forms
Employer, FMLA, insurance forms, or any other paperwork that requires your physician’s input, can be very time consuming for you and your physician. Please be sure to complete all pertinent information before asking us to review and complete the physician’s part of the form. Fees start at $25 for completion of forms.
Referrals
A referral from your Physician may be made to an outside specialist. If your insurance does not require a referral from your primary care provider, you may contact that specialist office directly to make an appointment. If your insurance does require authorization from your primary care provider, you may be asked to make an appointment before the referral can be issued.
Requests for Medical Records
We will release copies of a patient’s medical record with written, signed patient authorization. The fee is based on the number of pages. You will not be charged a fee for records requested by a physician to whom we have referred you.
Worker’s Compensation
We do not file worker’s compensation claims.
Termination from this Practice
We value our patient relationships and want to protect patients’ rights. We will only terminate patient relationships after careful consideration and with legitimate reasons. Reasons for termination include: repeated no-showing for scheduled appointments, not complying with recommended medical care; being hostile or abusive to physicians or staff; or not paying bills in a timely way. We also reserve the right not to accept certain medical conditions if we believe that better care can be provided at another facility.
Health Insurance Portability and Accountability Act (HIPPA)
The federal government requires us to share our Privacy notice, which is posted in our office. Please review the Privacy Notice, which explains our policy on sharing patient information for treatment and billing issues.
